Our Health Center Locations

Cascadia has four health centers in the Portland metro area. Each health center offers mental health, primary care, and substance use disorder (SUD) services. For additional services offered at each health center, see below.

Cascadia also has a Mental Health Urgent Walk-In Clinic, open 365 days a year, located in the same building as our Plaza Health Center.

To access Cascadia’s services, please call the Cascadia Access, Information, and Referral line at 503-674-7777, Monday through Friday, 9:00 a.m. to 5:00 p.m.

Masks are encouraged but not required at all of Cascadia’s locations.

Garlington Health Center

The Garlington Campus is one of Oregon’s most innovative community-centered health care campuses. The campus is anchored by the Garlington Health Center, which opened in 2018 and provides a suite of integrated health care services, all in one location, to support a person’s whole health needs.
